Tom Twelftree is a healthcare technology executive with deep experience in digital transformation, patient flow, and community‑based service delivery across Australia and New Zealand. He is the founder and owner of Community Data Solutions (CDS), an Australian health and human services software company that has supported community, disability, and social care organisations to modernise case management and operational reporting. As Chief Executive Officer of Community Data Solutions, Tom led the company’s growth from a niche provider into a trusted technology partner for community and health organisations across multiple jurisdictions. Under his leadership, CDS developed a strong reputation for practical, outcomes‑focused software, close customer engagement, and a deep understanding of frontline service delivery. This positioning ultimately led to the successful sale of Community Data Solutions to VitalHub, a global healthcare software group specialising in patient flow, access to care, and clinical decision support.  Following the acquisition, Tom transitioned into a senior leadership role within VitalHub and now serves as General Manager for Australia and New Zealand, with responsibility for regional strategy, commercial performance, customer relationships, and delivery across the VitalHub product portfolio. In this role, he oversees the expansion and integration of VitalHub’s solutions across hospitals, health services, and community‑based care providers, ensuring alignment with local health system priorities and regulatory environments.  Tom brings a practitioner’s perspective to digital health leadership, combining commercial acumen with a strong focus on real‑world implementation and change management. He is known for building high‑trust partnerships with health executives, clinicians, and operational leaders, and for translating complex system challenges into scalable, technology‑enabled solutions. Based in Australia, Tom Twelftree continues to play a key role in shaping VitalHub’s presence and growth in the ANZ region, supporting health systems to improve access, efficiency, and patient outcomes through connected, data‑driven platforms
